Access the form by clicking on the "Download" button. A new window will open the pdf file. ​

Print the file:

• Windows users - Press the "Control" key (lowest left bottom key in the corner of most keyboard.) and then the "P" key.
   

• Mac users - Press the "Command" key (lowest row, just left of the space bar) and then the "P" key.

To save the file to your computer: 

1. Right-click on the form.
    

2. Select "Save As" and either keep the document title as it is or give it a new name.
    

3. Save the form on your computer, making a note of that location.

To open the file:

1. Open Adobe Reader.
    

2. Select "File" from the menu and then select "Open" in the dropdown menu to locate the form in your computer.
    

3. Double-click on the form name to open the file in Adobe Reader.
    

4. Print using the same instructions as above.
